The everyday gap

Care can be shared. The coordination often is not.

When updates live everywhere

Families piece together texts, calls, notes, and calendars while trying to stay ahead of appointments, medication routines, and changing needs.

CareBridge creates a common view

One shared place for the practical plan and the human context—so family members and care teams can spend less energy relaying information and more energy showing up well.

A steadier rhythm

The essentials, gathered around the people involved.

A shared care calendar

Put visits, appointments, and practical handoffs in one view so the next step is easier to see—and easier to share.

Designed for the coordination around care, not clinical documentation.

Care updates that reach the right people

Share everyday observations, reminders, and follow-ups without repeating the same update across channels.

Medication and appointment tracking

Keep the practical details visible for the people helping coordinate the week.

AI family summaries, built for clarity

Turn a busy stretch of updates into a concise recap that helps family members understand the week and prepare for the next conversation.

For home-care agencies

Invite families into a more connected care experience.

Your team already does the work of keeping care moving. CareBridge is a lightweight companion for the coordination and communication families feel most deeply.

Pilot with intention

  1. 1Choose a small, thoughtful group of care teams and families.
  2. 2Start with a focused coordination workflow that matters to them.
  3. 3Share feedback that helps shape CareBridge around real care relationships.

CareBridge is not an EHR and does not replace your agency's existing clinical or operational systems.

Questions, answered

A thoughtful start, without overpromising.

Who is CareBridge for?

CareBridge is being shaped with adult children coordinating support for an older loved one and home-care agencies looking for a clearer way to keep families in the loop.

Is CareBridge an electronic health record?

No. CareBridge is a shared care-coordination workspace for plans, updates, and everyday follow-through. It is not designed to replace an agency’s clinical, payroll, or record-keeping systems.

What does an agency pilot include?

Pilot partners help test the family experience with a small group of care teams. We will align on a focused workflow, listen closely, and use what we learn to improve the product together.

Can a family join without an agency?

Yes. We welcome families who want to help shape a calmer way to coordinate care. Tell us a little about your situation and we will follow up as early access opens.
